Record Unsupported Claims and Escalate Decisions

Turn an uncertain AI output into a specific question that the responsible reviewer can resolve.

An assistant can produce a convincing assertion without support. Do not decide whether to trust it from tone or confidence. Keep the output and source packet; record each disputed assertion, source gap, consequence, responsible reviewer and next action.

Checks before use

  1. Reopen the relevant source and compare the exact wording. Separate a missing source, conflicting sources and an interpretation that requires legal judgment; they need different follow-up.

  2. Pause the affected conclusion or external action when a material fact, authority, deadline trigger or approval is unresolved. Continue independent work only if the open issue does not affect it.

  3. Send the responsible reviewer the assertion, both passages if sources conflict, why the issue matters and the specific decision needed. Record the resolution and recheck every dependent passage before use.

Synthetic worked example and correction

Synthetic records show dispatch on Monday and no evidence of receipt. The draft declares timely service. The reviewer marks receipt and the applicable service rule unresolved, assigns the evidence and procedural questions, and holds the service conclusion. The dispatch record alone does not establish which service rule applies or whether its requirements were met.
